Six vehicles were involved in a crash on Southside Boulevard near Ivey Road on Thursday night that sent five people to hospitals with serious injuries, according to the Florida Highway Patrol.

Troopers said the accident happened about 7 p.m. when the driver of a Cadillac headed south lost control of the car, crossing the grass median, then into a left-turn lane, where it rear-ended a Toyota Tundra pickup truck.  The Cadillac continued into the northbound lane where it struck a BMW head on, which sent the BMW into a Honda Civic.

A Toyota Prius attempted to avoid the crash, but hit the rear of the Civic, then the Prius was rear-ended by a Honda Accord.

The driver of the Cadillac -- Erica Chambers, 22, of Jacksonville -- and the 2- and 3-old children in the car were all injured in the wreck and taken to Shands Jacksonville Medical Center, according to the FHP report.  Troopers also noted that neither of the children was restrained by a seat belt or safety seat.

Troopers say the driver of the Civic and a 14-year-old in the Prius were also seriously injured.  The driver and passenger in the BMW, the driver and an 18-month-old in the Prius and a passenger in the Accord were listed as having minor injuries.

The FHP says charges are pending the outcome of the investigation.
